WEBINAR ON DEMAND
Process Improvement for RTH and RTED Reduction

IMMPACT in partnership with Lumina Learning Institute (LLI) has embarked on providing CE (Continuing Education) credits to nurses. Recently, we have implemented some process improvement protocols for nursing homes and post acute care facilities that has improved the Return To Hospital (RTH) and Return To ED (RTED).

This is time that we engage all the nurses to understand the process and reason behind it. This program was recorded and is available in our clinical education vault for future registered clinical audiences. For a limited time, this program is included at no additional cost for staff at facilities under IMMPACT's medical directorship.

This is another big step forward in our QAPI (Quality Assurance and Performance Improvement) process with your active participation and partnership.

After watching this recorded webinar you will:

  • Understand the importance of creating a priority list of high-risk residents
  • Learn the value of structured communication, including the escalation process, with residents
  • Be able to list best practices for patient care communication with the hospital ED
